CPF Nomination (5 minutes to start)
Decide how you'd like your CPF savings to be distributed when you pass on. All you need is two persons to witness your nomination online. ➡️ Start now (opens in new tab).
Lasting Power of Attorney (LPA) (10 minutes to start)
Appoint trusted persons to make decisions on your behalf in the event that you lose mental capacity. These decisions include your personal welfare, property and bank accounts.

Advance Care Planning (30 minutes to start)
Share your preferences so that your doctors and those you trust can tailor your care and treatment accordingly, when you are not able to speak for yourself. This includes indicating the activities you enjoy, and worries you may have about your medical care.
Step 1: Use the myACP (opens in new tab) service to prepare an Advance Care Plan (ACP) online.
Step 2: Get each Nominated Healthcare Spokesperson (NHS) to accept their appointment.
Step 3: Share ACP with NHS once registered
Alternatively, you may schedule an appointment with an ACP facilitator (opens in new tab) to register your plan. You can meet your facilitator online or in person.
