Legacy planning helps make your wishes known by giving you the opportunity to decide who can act on your behalf, communicate your care preferences, and ensure your assets are distributed according to your wishes.

Your legacy is yours to define. Make sure your voice is heard, even when you’re not there to speak. Planning ensures your wishes are respected, leaving no room for doubt. Take the first step to shape the future on your terms. Take action with these tools.

Lasting Power of Attorney (LPA)

Purpose: Appoint someone you trust to make decisions on your behalf in the event that you lose the mental capacity to do so.

How it makes your wishes known: Provide clarity on whom you trust to make decisions on your welfare, property and finances when you are not able to speak for yourself.

What to do:

  • Step 1 - Consider whom you wish to appoint to make decisions on your behalf (called Donees) and the powers to be granted to them.

  • Step 2 - Complete the online LPA form (opens in new tab) (10 minutes).

  • Step 3 - Your Donee(s) will receive SMS notifications to accept the appointment(s).

  • Step 4 - After your Donee(s) have accepted the appointment(s), schedule an appointment with a Certificate Issuer (opens in new tab) to certify your LPA.

  • Step 5 - Meet your Certificate Issuer, who will ensure you understand the purpose of the LPA and the scope of authority you have conferred on your trusted persons.

Advance Care Planning (ACP)

Purpose: Record your healthcare preferences for potential end-of-life or mental incapacity situations.

How it makes your wishes known: Make your medical preferences explicit, so that your doctors and those you trust can tailor your care and treatment in a way that respects your preferences, when you are not able to speak for yourself.

What to do:

  • Step 1 - Use the myACP (opens in new tab) service to prepare an Advance Care Plan (ACP) online.

  • Step 2 - Get each Nominated Healthcare Spokesperson (NHS) to accept their appointment.

  • Step 3 - Share ACP with NHS once registered.

Alternatively, you may schedule an appointment with an ACP facilitator (opens in new tab) to register your plan. You can meet your facilitator online or in person.

CPF Nomination

Purpose: Specify how you'd like your CPF savings to be distributed when you pass on.

How it makes your wishes known: Clearly communicate your intended beneficiaries to ensure your wishes are respected.
