Lasting Power of Attorney (LPA)

Purpose: Appoint trusted persons to make decisions on your behalf in the event that you lose mental capacity.

How it brings peace of mind: Knowing someone you trust has been given the legal right to act in your best interests will reduce your worries about the future.

What to do:

  • Step 1 - Consider whom you wish to appoint to make decisions on your behalf (called Donees) and the powers to be granted to them.

  • Step 2 - Complete the online LPA form (opens in new tab) (10 minutes).

  • Step 3 - Your Donee(s) will receive SMS notifications to accept the appointment(s).

  • Step 4 - After your Donee(s) have accepted the appointment(s), schedule an appointment with a Certificate Issuer (opens in new tab) to certify your LPA.

  • Step 5 - Meet your Certificate Issuer, who will ensure you understand the purpose of the LPA and the scope of authority you have conferred on your trusted persons.

Advance Care Planning (ACP)

Purpose: Communicate your healthcare and personal care preferences for a situation where you lose mental capacity or cannot speak for yourself.

How it brings peace of mind: Know that your preferences will be communicated to your doctors and loved ones, and decisions made will be in your best interest.

What to do:

  • Step 1 - Use the myACP (opens in new tab) service to prepare an Advance Care Plan (ACP) online.

  • Step 2 - Get each Nominated Healthcare Spokesperson (NHS) to accept their appointment.

  • Step 3 - Share ACP with NHS once registered.
